Classic work hours (occasional evening or weekend client visits) CDHD is looking for a dynamic and experienced Public Health Nurse - NFP Nurse Home Visitor to join a team dedicated to providing comprehensive nursing services to women and their families eligible for the Nurse Family Partnership Program.
Involves extensive interaction with socio-economic and culturally diverse populations of clients, community-based agencies, medical personnel, and district staff.
The ideal candidate will have knowledge and experience working in the public health sector as a community health nurse.
A snapshot of your responsibilities Serves a caseload of up to 25 pregnant women and their children, with the goal of improving maternal & childhood outcomes and increases economic self-sufficiency of program participants.
Provides home visits to women and their families under NFP program.
Adheres to nursing process and NFP model of home visitation.
Participates in community coalitions in support of Nurse-Family Partnership.
Participates as a member of local interagency committees, plans meetings with relevant local service and civic groups and participates in the development of new community resources related to areas of assignment.
Works with the NFP team on continual improvements to program development, implementation and evaluation.
What we require BS in nursing from an accredited university, which includes a community health component.
Current CPR Certificate RN Licensure - must have a valid license to practice as a registered professional nurse in the State of Washington, If not must obtain a license by date of hire.
Minimum of two (2) years of public health or community-based nursing experience.
Continuing education required per RN Licensure.
Strong computer knowledge required including Word, Excel, PowerPoint and Outlook.

Estimated Salary: $20 to $28 per hour based on qualifications.
